The Medical Records Department is staffed with a group of employees dedicated to help and serve you. We understand that it can be very stressful to take care of your health needs and also take care of the paper work that may be involved. We will do our part to make your experience with The Orthopaedic Group, a division of Connecticut Orthopaedic Specialists, a positive one.

A written request and a completed authorization is required for the release of a patient’s medical record. (Please see section below for instructions on how to print out these forms).

All patient records requests MUST have a completed and signed HIPPA compliant authorization BEFORE a request can be processed. All requests will be processed within 30 days of receipt.

Please allow us a minumum of 5-7 business days to initiate the process once you have requested records.

A fee of $.65 cents per page may be charged for copies of Medical Records.

Our HIPAA Notice of Privacy Practices describes how medical information about you is used and disclosed, and also how you can get access to that information.

How to Request Medical Records

To request a copy of a patient’s medical record, a HIPAA compliant authorization must be completed by the patient. Please click here to access our Authorization for Use and Disclosure of Protected Health Information. The patient must fill out the authorization completely sign and date and either fax back to 203-401-0035 or mail it to: The Orthopaedic Group, a division of Connecticut Orthopaedic Specialists, Attn: Medical Records Dept., 9 Washington Ave, First Floor, Suite 1-A. Hamden, CT 06518.

Disability and Benefit Forms

We can process Disability and Benefit forms that are related to your orthopedic treatment and care. There is a $20 processing fee (exceptions: Social Security forms and Workers Compensation forms)

  • Family Medical Leave Act (FMLA) forms- processing fee
  • Social Security Forms (S.S. benefits) - no fee
  • Workers Compensation forms- no fee
  • Aflac forms- processing fee
  • Insurance disability forms- processing fee

A completed copy of our Authorization for Use and Disclosure of Protected Health Information must be included. All forms can be mailed to the above address along with a completed authorization and the $20 processing fee. Once received, we will process the form, and mail it back to the patient or directly to the company requesting the information within one week.

If you require copies of your xray films/or onto a CD, there will be a charge.
